Forms

This waiver is required for all attendees by the campground. Eastside Foursquare is the church that is on the contract for the campground. All churches, attending Camp Genesis will fill out the same campground waiver! This must be submitted before Wednesday, June 24 or your student will not be able to attend camp.

Students are required to complete the Parental Consent & Release Form, which covers medical history, emergency treatment authorization, risk acknowledgment, and photo release; the Student Medication Form, which lists all current medications, allergies, and medical history; and if transportation arrangements differ from standard drop-off and pick-up, the Transportation Waiver & Parental Release Form must also be completed.

Adult Leaders are required to complete the Adult Leader Declaration & Release Form, which covers accident and insurance coverage, camp conduct guidelines, background check authorization, risk waiver, and photo release. A pastor signature confirming a current background check (within the last 3 years) is required before camp.

For all forms, please print, complete, sign where indicated, and turn in to your Youth Pastor before camp.

PACKING LIST

We recommend students bring the following:

  • Bedding (Sleeping bag, pillow, blankets)

  • Linens for twin size bed (optional)

  • Bible, journal, & pen

  • Toiletries (toothbrush, toothpaste, shampoo/conditioner, body wash, deodorant, etc)

  • Towels

  • Sunscreen

  • Modest clothing for 5 days/4 nights (shirts, underwear, socks, etc)

  • Modest swimsuit (one piece, tankini, or t-shirt on top of two pieces required for girls, swim trunks and a t-shirt when not in the water for boys)

  • Shoes (athletic, sandals, shower shoes)

  • Water Bottle

  • Color team spirit gear

  • Flashlight or headlamp  

  • Medications, vitamins, medical supplies stored in a gallon ziploc bag that will be left with the camp nurse at check-in 

  • A snack to share (Think 5-10 people)

  • Optional: Money for the snack shack, coffee bar, or camp merch
